Interim Healthcare Services of Nepa Is Looking for an Experienced Registered Nurse (Rn) to Join Our Growing Team. the Ideal Candidate Will Be Comfortable Working Independently, One-on-one with Clients in a Home Bound Setting. Must Possess One (1) Year Experience As a Registered Nurse.

- Wyoming and Susquehanna Counties
- Monday Through Friday 8:30 Am to 5:00 Pm
- On Call Weekend and Weekday Rotations

The Registered Nurse (Rn) Provides Services and/or Treatments Requiring Substantial and Specialized Nursing Skill and Provides Effective and Efficient Patient Care in Accordance with Applicable Law and Regulations, Accepted Standards of Nursing Practice, Payor Requirements and Interim Healthcare Policies and Procedures.
Essential Functions of the Registered Nurse Rn - Home Health:

Patient Assessment & Care Planning: Conducts Comprehensive Initial and Ongoing Assessments; Collaborates with Patients, Caregivers, and the Interdisciplinary Team to Develop and Evaluate Individualized Care Plans Based on Mutually Agreed Goals.

Care Implementation: Delivers Physician-ordered Services, Including Preventive and Rehabilitative Procedures, While Adhering to Nursing Standards and Clinical Guidelines.

Patient & Family Support: Educates and Counsels Patients and Families on Healthcare Needs to Support Informed Care and Recovery.

Clinical Documentation: Maintains Timely, Accurate, and Thorough Clinical Notes, Tracking Patient Progress and Outcomes.

Communication & Coordination: Effectively Communicates with Healthcare Team Members and Physicians to Coordinate Care, Address Changes, and Plan for Patient Discharge.

Quality & Compliance: Participates in Quality Assessment and Performance Improvement (Qapi) Initiatives; Ensures Compliance with Professional Standards and Organizational Policies.

Clinical Decision Making: Applies Sound Judgment to Manage Care Efficiently and Adapt to Patient Needs.

Additional Duties: Performs Other Duties Within Scope of Licensure and Responsibly Handles Access to Personal Health Information (Phi) in Accordance with Regulations.
Minimum Requirements for the Registered Nurse Rn - Home Health:
- Nursing License in the State of Pennsylvania
- One (1) Year Experience in Home Health, Hospice, or Comparable Field; Required.
- Proof of Cpr Training; Required.
- Reliable Form of Transportation
- Willing to Undergo Pre-employment Screenings.
