Responsibilities:

Provide compassionate, skilled 1:1 nursing care in the home

Collaborate with RN Supervisor on the care plan

Recognize and report patient condition changes

Educate patients and families on care and prevention

Document timely and accurately

Follow accepted standards of nursing practice
