Schedule
-
Monday through Friday, 8:30 AM–5:00 PM
-
Some on-call, weekend, and holiday rotation
What You’ll Do
-
Deliver one-on-one patient care in a home health setting
-
Complete initial and ongoing assessments
-
Develop and update individualized plans of care
-
Communicate effectively with physicians, caregivers, and families
-
Educate patients on health management and recovery
-
Ensure timely and accurate documentation in compliance with regulations
What We’re Looking For
-
Active RN license in the State of Pennsylvania
-
Minimum 1 year of Home Health or Hospice experience
-
CPR Certification (required)
-
Reliable transportation and valid driver’s license
-
Compassionate, dependable, and committed to excellent patient care
