🩺 What You’ll Do
As a Home Health Physical Therapist, you’ll help patients regain mobility, confidence, and independence through personalized, one-on-one care in their homes.
- Conduct physical therapy evaluations and OASIS assessments
- Develop and implement individualized care plans to restore function and mobility
- Deliver hands-on therapeutic interventions and home exercise programs
- Educate patients and caregivers on proper exercise, movement, and safety techniques
- Collaborate with nurses, physicians, and other care team members to optimize outcomes
- Maintain accurate and timely clinical documentation
✅ What You’ll Bring
- Active New Jersey Physical Therapist (PT) license
- Master’s or Doctorate in Physical Therapy
- Home care experience (required)
- CPR certification, valid driver’s license, reliable transportation, and insurance
- Excellent communication and organizational skills
- Passion for providing personalized, in-home rehabilitative care
