Schedule
-
Monday–Friday, 8:30 AM–5:00 PM
-
Some on-call, weekend, and holiday rotation
What You’ll Do
-
Provide skilled, one-on-one nursing care in patient homes
-
Complete initial assessments and routine reassessments
-
Develop, implement, and update personalized care plans
-
Communicate closely with physicians, families, and caregivers
-
Educate patients on disease management and recovery
-
Document care accurately and on time according to home health standards
What We’re Looking For
-
Active RN License in Pennsylvania
-
At least 1 year of Home Health or Hospice experience
-
CPR Certification
-
Reliable transportation & valid driver’s license
-
A compassionate, dedicated nurse with strong clinical and communication skills
