🩺 Role Responsibilities
- Provide skilled nursing care per physician orders, agency policy, and regulatory guidelines
- Perform OASIS assessments and documentation from admission through discharge
- Update and re-evaluate plans of care at least every 60 days (or as required)
- Act as case manager, coordinating multidisciplinary care (nursing, home health aides, therapy, social services)
- Maintain accurate, timely, professional documentation of assessments and care delivery
- Participate in the agency’s Quality Assurance and Performance Improvement (QAPI) programs
- Ensure all care meets compliance and regulatory standards
👤 Ideal Candidate Profile
- Must hold active NYS Registered Nurse license
- Valid NY Driver’s License, CPR/BLS certification, and COVID-19 vaccination (as applicable)
- 1+ year RN experience preferred; new graduates welcome — agency offers onboarding support
- Previous home care, acute care, or rehab experience is a plus
- Familiarity with OASIS assessments is beneficial but not required
