We are currently seeking a caring and dependable Registered Nurse (RN) to join our Home Health team serving Portsmouth, NH and surrounding communities. If you are passionate about patient-centered care and looking for a flexible and supportive work environment, this opportunity is for you.
Responsibilities
- Provide one-on-one skilled nursing care to patients in their homes
- Assess patient conditions and develop individualized care plans
- Administer medications, treatments, and other nursing interventions as prescribed
- Monitor patient progress and communicate changes to physicians and healthcare team members
- Educate patients and family members regarding treatment plans, medications, and self-care techniques
- Maintain accurate and timely clinical documentation
- Collaborate with interdisciplinary healthcare professionals to support patient goals and quality outcomes
- Ensure compliance with all state, federal, and company regulations and standards

Qualifications
- Current Registered Nurse (RN) license in the State of New Hampshire
- Minimum of one (1) year of nursing experience within the last five years preferred
- Home health or home care experience preferred
- Current CPR Certification required
(Monthly renewal classes available and paid for by Interim HealthCare)
- Strong communication and clinical assessment skills
- Compassionate, dependable, and patient-focused approach to care
