Schedule: Part-Time / PRN (Flexible Scheduling Available)

Job Overview:
We are currently seeking a compassionate and skilled Home Health Occupational Therapist – OT to provide personalized, one-on-one therapy in a home setting throughout Collinsville, Va and the surrounding areas. This position is ideal for an Occupational Therapist who enjoys working independently while helping patients improve mobility, coordination, and independence in their daily environment.
As a Home Health Occupational Therapist – OT, you will work directly with patients recovering from illness or injury while developing individualized treatment plans focused on improving patient outcomes, safety, and quality of life within the home.
Responsibilities of Home Health Occupational Therapist – OT:
- Provide occupational therapy to patients unable to perform daily tasks due to illness or injury
- Conduct patient assessments and establish individualized therapy goals
- Monitor patient progress and modify care plans as needed
- Assist patients with exercises to improve fine motor skills and coordination
- Recommend adaptive equipment such as grab bars, shower chairs, and other safety tools
- Assess fall risks and implement strategies to improve home safety
- Educate patients and families on therapy goals, exercises, safety practices, and self-care techniques
- Collaborate with interdisciplinary team members including RN, LPN, CNA, HHA, PT, and SLP
- Maintain accurate and timely documentation of patient progress
Qualifications of Home Health Occupational Therapist – OT:
- Graduate of an accredited Occupational Therapy program
- Active OT license in the state of Virginia
- CPR certification and TB test required
- Knowledge of state and federal home health regulations
- Strong clinical judgment, interpersonal, and critical thinking skills
- Compassionate, dependable, and patient-focused
- Ability to work independently in a home setting
