As a Home Health Physical Therapist, you'll help patients recover safely and confidently in their own homes by:
- Evaluating patients and developing individualized treatment plans
- Restoring strength, mobility, balance, and functional independence
- Providing hands-on therapy and therapeutic exercise
- Monitoring progress and adjusting care plans as needed
- Collaborating with nurses, occupational therapists, speech therapists, home health aides, and other clinicians to coordinate care
- Educating patients and family members on exercises, safety strategies, and ongoing recovery
- Completing accurate and timely clinical documentation
What We're Looking For
- Graduate of an accredited Physical Therapy program
- Active New Hampshire Physical Therapist license
- Minimum of one year of PT experience (home health experience preferred)
- Current CPR certification
- Knowledge of state and federal home health regulations
- Excellent clinical judgment, communication, and patient education skills
- Compassionate, organized, and committed to delivering exceptional patient care
