Schedule
-
Monday–Friday, 8:30 AM–5:00 PM
-
Some on-call, weekend & holiday rotation
What You’ll Do
-
Deliver one-on-one skilled nursing care in patient homes
-
Perform initial assessments and ongoing evaluations
-
Develop & update individualized care plans
-
Collaborate with physicians, families, and caregivers
-
Educate patients on managing their health
-
Complete documentation accurately and on time
What We’re Looking For
-
Active RN License in Pennsylvania
-
Minimum 1 year Home Health experience
-
CPR Certification
-
Reliable transportation & valid driver’s license
-
A compassionate, dependable nurse who takes pride in their work
