The Performance Improvement Specialist is responsible for the clinical and quality review of care plan and assessment documentation for best practice planning, optimal quality outcomes and utilization statistics utilizing the EMR (Home Care Home Base) Strategic Healthcare
Programs (SHP) and Oasis Assessments.
This position will provide education to the clinical teams utilizing multiple modalities including group sessions/team meetings and 1:1 education on quality metrics, outcomes, processes measure and utilization statistics, guidance of best-practice care planning and optimal utilization of services; providing twice a month episode management conferences and education of multi-disciplinary teams in the management of patient care services. The Performance Improvement Specialist reviews utilization statistics and adverse events, including the study of hospitalized patients; seeking patterns and providing correlating opportunities to mitigate preventable risk; setting ongoing expectations for standardized, risk-stratified, best practice care planning.

Qualifications:
- Clinical Professional such as a Registered Professional Nurse, Physical Therapist.
- Bachelor’s Degree in discipline required, Master’s Degree preferred.
- Minimum of 5 years of home care experience
- OASIS Certification is required.
- License in clinical profession.
- Previous leadership experience desired
- Flexible in scheduling and hours worked and innovative
- Consistently seeks resources to educate self and others on evidence based best practices initiatives (such as methodology to reduce avoidable hospitalization and improve specific clinical outcome scores.)
- Committed to the provision of quality of care
- Skilled communicator and team builder
- Able to work with a diverse population and skill level of staff.
- Able to function independently and be a self- starter.
