We’re seeking a dedicated and motivated Physical Therapist (PT) to join our growing team and help patients improve their mobility, independence, and overall quality of life.

💼 What You’ll Do as a Physical Therapist (PT)

Deliver physical therapy services in accordance with physician’s plan of care and state PT practice guidelines
Assess and evaluate patients’ therapeutic, rehabilitative, and functional status
Develop and direct individualized therapy programs focused on rehabilitation and recovery
Evaluate home environments and recommend safety/therapy modifications
Train patients and caregivers in therapeutic appliances and prosthetic device use
Provide instruction and support to families/caregivers and collaborate during case conferences
Supervise and guide Physical Therapy Assistants (PTAs) per policy and regulations
Monitor and report patients’ responses to treatment, adjusting plans as needed
Prepare clinical and progress summaries, participate in discharge planning, and coordinate with interdisciplinary teams

🎯 What We’re Looking For

Active Physical Therapy license in Indiana and/or Ohio
Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program
CPR certification and recent TB test required
Valid driver’s license, reliable transportation, and auto insurance
2+ years of physical therapy or rehab experience preferred (community/home health experience ideal)
Strong communication, organizational, and leadership skills
Ability to build rapport with patients and handle physical workload
