Interim Healthcare of Scranton is hiring a Home Health Physical Therapist (PT) for adult patients in Wayne & Pike Counties. This is a W2 role with a realistic daily schedule and 1:1 patient care.

Responsibilities

Provide in-home PT evaluation and treatment

Create and update individualized plans of care

Document patient progress and outcomes

Teach patients and caregivers exercises + safety strategies

Coordinate care with the interdisciplinary team
