VNSW AT HOME. The Home Health Aide (HHA) is a member of the health team who provides supportive personal care services to patients in their homes, under the guidance and supervision of the nurse.
General Duties (each patient has specific care needs, skills/job duties subject to change)
1. Provides assistance with personal care which includes: Bathing, Grooming, Feeding, Dressing, Assist with walking, use of walker, cane or wheelchair
2. Assists with other functions as assigned and instructed by the nurse or therapist, Assist with transfers from bed to chair or commode and back, Ambulation with and without assistive devices, Maintaining simple records as requested
3. Assists with secondary duties related to the client’s environment
4. Documents care as appropriate and on a timely basis
5. Reports significant changes in the patient or his/her care to the nurse as soon as possible.

Requirements:
- Current certification as a Home Health Aide from a New York State approved training program
- Valid/Current identification to satisfy I-9 requirements
