The Program Specialist plays a central role in ensuring the faithful and effective implementation of Individual Support Plans (ISPs) for individuals with IDD. By monitoring service delivery, tracking outcomes, and facilitating communication between families, Supports Coordinators, and direct service teams, this role ensures that every goal, safeguard, and service outlined in the ISP is both delivered and documented in compliance with 55 Pa. Code § 6100.411 and § 6100.221–226.
- Monitor ISP implementation across all domains and ensure services reflect the person-centered strategies approved in each individual’s plan.
- Conduct monthly and quarterly service reviews, document outcomes, and update progress toward goals.
- Participate in annual ISP meetings, interdisciplinary planning, and incident debriefings.
- Visit individuals in their homes to verify services, review documentation, and assess satisfaction.
- Collaborate with Behavior and Nursing teams to ensure clinical recommendations are embedded in daily routines.
- Provide coaching and support to direct care staff on proper goal tracking and documentation practices.
- Track behavioral, medical, and social service outcomes to identify trends or areas for adjustment.
- Ensure compliance with documentation standards per § 6100.227 and audit-readiness for licensing or QA&I.
- Submit and track incident reports, ensuring follow-up meets timelines and protocols per Bulletin 00-21-02.
- Facilitate communication with SCOs, family members, and clinicians to align service delivery with changing needs.
- Maintain organized service documentation, including logs, assessments, progress notes, and regulatory forms.
- Support new referrals and transitions by ensuring timely ISP implementation and service alignment.
Must meet one of the following per § 6100.411:
- Master’s degree + 1 year IDD experience
- Bachelor’s degree + 2 years IDD experience
- Associate’s degree + 60 credits + 4 years IDD experience
- Knowledge of ISP development, HCBS Waiver service definitions, and incident management.
- Strong written communication, coaching, and person-centered planning skills.
- Clearances: PA FBI fingerprint, Child Abuse History, and State Criminal Background.
- Valid driver’s license and reliable transportation.
