Job Summary
The Agency Supervisor ensures the quality of patient care aligns with WE C.A.R.E.'s objectives and policies. This role involves planning, coordinating, implementing, and evaluating home health services while providing daily supervision and support to agency staff.
Key Responsibilities
- Coordinate and schedule daily workloads to maximize personnel capacity.
- Supervise district nurses and Home Health Aides to maintain service standards.
- Evaluate clinical content, diagnoses, medical history, and procedures in patient records.
- Guide staff on appropriate use of community resources.
- Conduct performance appraisals and provide counseling for professional development.
- Review patient needs and ensure appropriate care assignments.
- Assist with staff orientation and community representation.
- Conduct audits, interdisciplinary team conferences, and represent the agency as requested.
- Oversee nurse schedules and manage patient re-hospitalization documentation.
- Perform additional tasks as directed by the Administrator or Clinical Manager.
Qualifications
- Minimum of 2 years of nursing experience, including 1 year in a supervisory role within a Home Health Agency or similar facility.
- Strong clinical decision-making skills and QAPI process knowledge.
- Current CPR certification.
- Ability to meet physical requirements, including occasional travel and patient care in non-accessible environments.
