Hiring Locations
We are actively hiring Physical Therapists (PTs) for home care opportunities in:
- Westchester County, NY
- Rockland County, NY
- Bronx County, NY
Key Responsibilities
- Assessment: Evaluate patients’ functional limitations and develop personalized therapy plans with patients, families, nurses, and physicians.
- Direct Care: Deliver therapy services using accepted physical therapy modalities for medical and surgical conditions.
- Education:Â Train and supervise patients, families, and home health aides on exercise programs and care techniques.
- Ongoing Evaluation: Monitor and adjust therapy plans as patients’ conditions evolve.
- Communication & Documentation:Â Report updates to physicians and managers, and maintain accurate clinical records of evaluations, treatments, and outcomes.
Qualifications
- Active New York State Physical Therapist (PT) license & registration (required)
- Minimum 1 year of full-time physical therapy experience
- Home care experience preferred (not required)
- Ability to use technology (tablets) for documentation; Oasis experience a plus
- Valid driver’s license and reliable automobile required for field visits
Compensation & Benefits
- Salary: $95,000 – $108,000 annually (based on experience)
