💼 What You’ll Do:
- Evaluate Patients In-Home: Perform assessments to determine functional limitations and mobility issues.
- Develop & Implement Care Plans: Collaborate with physicians, nurses, and families to design effective, personalized therapy programs.
- Provide Direct Therapy: Administer physical therapy using evidence-based techniques.
- Educate & Train: Teach patients, caregivers, and home health aides home exercise programs and safety practices.
- Track & Adjust Progress: Monitor patient response and adjust care plans accordingly.
- Document & Communicate: Maintain clear records and coordinate with your clinical team.
✅ What You’ll Need:
- Valid New York State Physical Therapist license and current registration
- Minimum 1 year of physical therapy experience (home care experience a plus)
- Familiarity with digital documentation and use of tablets
- Experience with OASIS documentation is preferred
- Reliable transportation and valid driver’s license
💰 Compensation:
- $100,000 to $116,000 per year, depending on experience and qualifications
